The Community Resource Corps (CRC) program is a new AmeriCorps project in New Hampshire, designed to improve health and health outcomes for individuals affected by substance use and behavioral health disorders. As a CRC member you will serve with a team of other dedicated, compassionate people who will serve in one of three NH regions. You’ll support the work of primary care clinics, behavioral health centers and other social service organizations by being part of a healthcare team. You’ll work with patients to identify some of the barriers in their daily life and help them identify services that address problems such as homelessness, hunger, transportation, childcare or financial needs. You’ll support the improved health of many people by increasing access to community services, supporting care coordination between nurses, doctors, and social workers, and connect patients to community resources they need.
